Quarterly Appraisal form
Overall yearly performance ratings shall be rated as follows:
5 - “Exceptional” This means that employee’s performance is consistently superior and significantly exceeds given targets.
4 - “Excellent” This means that employee’s performance frequently exceeds given targets.
3 - “Acceptable” This means that employee’s performance consistently meets given targets.
2 - “Fair” This means that employee’s performance consistently meets some but not all given targets.
1- “Unsatisfactory” This means that employee’s performance fails to meet minimum position requirements; employee lacks skills required, or fails to utilize necessary skills.
Please note for sales target, lead generation and lead conversion, you are expected to use these guide lines below as provided in your condition of service.
SALES TARGET / LEAD GENERATION / LEAD CONVERSION
(0 - 0) = 0 point / Very bad
(1 – 100,000) = 1 point / Poor
(100,001– 400,000) = 2 points / Average
(400,001 – 700,000) = 3 points / Good
(700,001 – 1,000,000) = 4 points / Excellent
(1,000,001 and above) = 5 points / Excellent
